Police Certificate Application Guidelines

 Documents required for application

Please prepare the following documents. You must submit your application in person.

Incomplete applications may not be accepted.

1 A document confirming the necessity of a Police Certificate

You must provide a document issued by a public authority, such as a foreign embassy or an immigration bureau, which confirms the request for a Police Certificate. This document should include your name, country, and the reason for the request.
  We will confirm the details of the document by telephone.

2 A valid passport

We do not accept photocopies. If you are planning to renew your passport, you must bring your new passport after renewal.

3 A document proving residency in Nagano Prefecture

① For registered residents of Nagano Prefecture
Any one of the following documents:
  ・An official copy of your Certificate of Residence(issued within the last six months)
  ・A Japanese driver’s license(the address and name must exactly match what appears on your Certificate of Residence, including kanji)
  ・A “My Number” card (notification cards will not be accepted)
  ・A Residence Card
  ・A Special Permanent Resident Certificate

② For individuals who are not registered residents of Japan, but whose last registered residence was in Nagano Prefecture
Please prepare both (A) and (B) below.
  (A) A document that confirms your overseas address
    A driver’s license issued by a foreign government, mail (a postal item) which lists your name and address, etc.
  (B) A document proving Nagano Prefecture as your last registered residence
 Any one of the following documents:
   ・An official copy of your Deleted Residence Record (issued within the last six months)
This can only be issued within 5 years of residence registration deletion
・A passport with a visa showing your stay in Japan, etc.

4 Handling fee

  400-yen Nagano Prefectural revenue stamp
 ※ Revenue stamps are not available at the Identification Division. They must be purchased in advance. Information on where to purchase revenue stamps is available on the official Nagano Prefecture homepage.
Please confirm that you have purchased the correct stamp. In Japanese, the stamp should read, “長野県収入証紙.”

  Monday-Friday, 9:00-12:00, 13:00-17:00
   (We are closed on Saturdays, Sundays, national holidays, and December 28 through January 5)
Please come during business hours. Reservations are not required.

◎ Where to submit your application

  Identification Division, Criminal Investigation Department, Nagano Prefectural Police Headquarters
  3916 Nishijo Matsushiro-machi
  Police Mobilization Headquarters 2F 
※ This is not the Nagano Prefectural Office

    
◎ Issuance of Certificates

  Certificates require about five business days to process. You or a proxy must return to the Nagano Prefectural Police Headquarters to receive your Police Certificate. Certificates cannot be mailed.
※ If holidays such as Golden Week or the New Year fall between the date of submission and expected date of issuance, your application may take longer than usual to process.
